Regulations for minors wishing to get tattoos

16 year olds
Both parents must sign a permission form/wavier
Both parents must present ID's
Minor must present a valid state issued ID
Minor must present an original birth certificate (NOT A COPY!)

17 year olds
Only ONE parent is required to sign a permission form/waiver
Parent must present a valid ID
Minor must present a valid state issued ID
Minor must present an original birth certificate (NOT A COPY!)
